流调中心抑郁量表(Center for Epidemiologic Studies Depression Scale, CES-D)

CES-D。共20题, 其中4 个为反向计分。要求受试用0~3级评定最近1周内症状出现的频度。总分为0~60分。原作者最初推荐使用16分作为可能存在抑郁问题的分界点, 对应80百分位; 后来建议以28 分作为抑郁状态的分界点, 对应95 百分位。

测量内容包含4 个因素:
抑郁情绪( depressed affect) , 含第1、3、6, 9、10、14、17、18题;
积极情绪( positive affect) , 含第4、8、12、16题;
躯体症状与活动迟滞( somatic and retarded activity) , 含第2、5、7、11、13、20 题;
人际( interpersonal) , 含第15、19题。

下列问题与你在过去一周里的感受和行为相关。请认真阅读量表中数字所代表的含义,然后标出最能说明你过去一周的感受和行为状况的数字。
0:几乎没有(少于一天)
1:很少有(1-2天)
2:经常有(3-4天)
3:差不多一直有(5-7天)

10.我感到害怕
I felt fearful.
11.我睡不安稳
My sleep was restless.
12.我感到快乐(反向评分)*
I was happy.
13.我讲话比平时少
I talked less than usual.
14.我觉得孤独
I felt lonely.
15.我觉得人们对我不友好
People were unfriendly.
16.我生活愉快(反向评分)*
I enjoyed life.
17.我哭过或想哭
I had crying spells.
18.我感到悲伤难过
I felt sad.
19.我觉得别人不喜欢我
I felt that people disliked me.
20.我提不起劲儿来做事
I could not get going.
